SAMS Forms for Broker NAID Registration
- Download both SAMS Form 1111 and 1111-A, be sure to sign
all forms. Line 22 for Form 1111. SAMS Form 1111-A requires
two signatures.
- Complete both forms along with the IRS Form W9
- Attach all required forms and include:
- A copy of Broker's License showing expiration/renewal
date
- A copy of a recent utility bill, bank statement
or a recent telephone bill is acceptable stating your current
business address.
- If using a Social Security number - Selling
Broker must provide copy of Social Security Card & driver's
license
- Or if using a Tax ID number - Selling Broker must complete
a W-9 and attach one of the following: EIN assignment letter,
payment coupon or tax paper containing your tax number.
- Return to BestAssets at the above address.
- The approval process may take 6-8 weeks.
Cannot process your application without proper
signatures on all forms
